Abstract
BACKGROUND: The present pilot study investigating the minimum dose for short-course single and double-dose treatment of kala-azar with an apparently new liposomal formulation of amphotericin B, Fungisome, led to identification of immunological components for early detection of success and/or failure to cure.Entities:

Plasma cytokine levels were measured by ELISA from all the 3 groups of patients, before treatment, 1 week after Fungisome treatment, at the time of relapse, and again after AmB treatment. Data are represented as mean (SEM). P values were calculated using Mann-Whitney U test for unpaired samples; P<0.05 was considered significant.
Significantly different from VL patients before treatment (P<0.005).
Significantly different from non-responders 1 week after Fungisome treatment (P<0.05).
Significantly different from the time of relapse (P<0.05).

NOTE. Leishmania-specific cell-mediated immune response in Fungisome treated patients. PBMCs were stimulated with 12.5 µg/ml LAg for 5 days and lymphoproliferation was measured by [3H]thymidine incorporation for the last 18–24 hours of culture. Cytokine levels were measured from supernatants of similar cultures after 96 hours calculated over background levels from all the 3 groups, before treatment, 1 week after Fungisome treatment, at the time of relapse and again after AmB treatment. Data are represented as mean (SEM). P values were calculated using Mann-Whitney U test for unpaired samples; P<0.05 was considered significant.
Significantly different from VL patients before treatment (P<0.05).
Significantly different from responders 1 week after Fungisome treatment (P<0.05).
*: Excluding 1 patient in Group A, who had very high level of TNF due to some unknown reason.
Figure 2Ability of Fungisome and AmB to modulate TNF, TGF-β and IL-10 production of human PBMCs.
PBMCs (1×106) from healthy controls were incubated with various concentrations of Fungisome and AmB with or without LPS (1 µg/ml) for 48 h at 37°C in 95% humidified air with 5% CO2. Cytokines (A) TNF, (B) TGF-β and (C) IL-10 were measured by ELISA from supernatants. Each symbol represents mean cytokine level (±SEM) at each dose of the drugs (n = 5). Values for p were calculated by ordinary one-way ANOVA followed by Tukey's multiple comparison tests; P<0.05 was considered significant. *, P<0.02–0.05; **, P<0.01.
