Abstract
The Cabrol procedure is one of several techniques used for re-implantation of a coronary artery. After replacement of the ascending aorta and aortic valve using a composite graft, second Dacron tube grafts are used for anastomosis between the ascending aortic graft and the coronary arteries. Ostial stenosis is one of the complications associated with the Cabrol operation. However, there have been no reported cases of acute thrombosis of a Cabrol graft. Here we report a case with acute ST elevation myocardial infarction due to thrombotic total occlusion of a right Cabrol graft-to-right coronary artery (RCA) twelve days after surgery in a patient with Marfan syndrome. He was successfully treated with primary percutaneous coronary intervention (PCI).Entities:

Fig. 1This coronary angiogram taken twelve days after the Cabrol graft operation revealed total occlusion by thrombi at the Cabrol graft to the right coronary artery anastomosis site (A: white arrow). After ballooning of the anastomosis site, it revealed a narrowed, angulated, and twisted distal limb of the graft anastomosis site (B). The coronary angiogram after stent implantation revealed complete reopening of the Cabrol graft to the right coronary artery (C).
Fig. 2Initial intravascular ultrasound and schematic illustrations showed a kinked and narrowed anastomotic site (A and B). After stent deployment, it showed optimal lesion coverage (C and D).
Fig. 3Schematic illustration of the Cabrol procedure showed that after replacement of the ascending aorta and aortic valve using a composite graft, a second Dacron tube graft is used to connect the ascending aortic graft and the coronary arteries (A). Coronary CT angiography shows that the left limb (black arrow) and the right limb of the Cabrol graft (white arrow) was kinked and narrowed at the anastomotic site (B and C).
