Prevalence and impact of information gaps in the emergency department.

Abstract

Information gaps may impact care of Emergency Department (ED) patients. We determined the incidence and consequences of missing information during 130 ED patient encounters. Among 15 encounters with missing information, 10 instances resulted in no reported consequences to the patient and 3 encounters resulted in an impact on patient care.
