Avian H4 not included as almost all subjects have antibody titers <1:10 against this virus (see Table 2). Sparse data were grouped when necessary to meet model assumptions. The proportional odds assumption was evaluated with the score test for proportional odds. Final multivariable models were designed using a saturated model including all potential risk factors (see methods) and manual backwards elimination. Age was forced into the final models. Flu shot was forced into the final models when no multicollinearity occurred.
