Lifestyle risk factors included cigarette smoking (ever), lack of physical activity (<30 min/day moderate to vigorous intensity activity), low diet quality (lowest three fifths of healthy diet score), alcohol intake of 0 or ≥15 g/day, and overweight (body mass index ≥25)
