Indications for Red Cell Transfusion
| To increase the oxygen-carrying capacity based on the hemoglobin level: | |
| Hb ≤60-70 g/L | Acute anemia: transfusion frequently required |
| Chronic anemia: transfusion may be necessary if the patient is symptomatic, there is limited cardiopulmonary reserve, or bleeding is present | |
| Hb >60-70 g/L but ≤100 g/L | Hb adequate for the majority of patients; transfuse only if the patient is symptomatic, is actively bleeding, or has limited cardiopulmonary reserve |
| Physiologic parameters in critical care: extraction ratio exceeding 50%, fall in | |
| Hb >100 g/L | Few patients require transfusion |
Hb, hemoglobin; PO2, mixed venous oxygen content; O2, oxygen consumption.
Conservative Indications for Prophylactic Platelet Transfusion
| Platelet Count × 109/L | Clinical Situation |
|---|---|
| 0-5 | Spontaneous bleeding likely, transfusion warranted |
| 6-10 | With fresh minor hemorrhage or fever, rapidly falling count |
| 11-20 | With concomitant coagulation disorder, heparin therapy, or planned minor invasive procedure |
| 21-50 | With major bleeding, surgical procedure |
| >50 | Bleeding unlikely to be caused by thrombocytopenia unless a qualitative platelet defect is present |
Frequency of Transfusion Reactions
| Complication | Estimated Frequency/Unit |
|---|---|
| Acute hemolytic transfusion reactions | 1/250,000-1/1 million |
| Delayed hemolytic transfusion reactions | 1/1000 |
| Anaphylaxis | 1/150,000 |
| Febrile, nonhemolytic reaction | 1/100-200 |
| Allergic/urticarial reaction | 1/200 |
| Transfusion-related acute lung injury | 1/8000 |
| Human immunodeficiency virus | 1/1.2-2.4 million |
| Hepatitis B | 1/60,000-1/200,000 |
| Hepatitis C | 1/800,000-1/1.6 million |
| Bacterial infection from red blood cells | 1/500,000 |
| Bacterial infection from platelets | 1/2000 |
