Age groups were selected pragmatically for sample size reasons rather than targeting special risk groups such as adolescents (15–19 years, n = 59).

A cluster adjusted chi-squared test was used to examine the significance of differences in proportions yielding wider confidence intervals and conservative P-values compared to the unadjusted chi-squared test. The differences in proportions had a similar pattern when analysis was undertaken excluding women who were currently pregnant (data not shown).
