Comparing two models of discharge planning rounds in acute care.

Abstract

Rounds are a primary part of the discharge planning process in acute care hospitals. The multidisciplinary aspect of rounds has been accepted as a given, but the differences resulting from differently composed interdisciplinary groups functioning in rounds has not been investigated. This experimental study examined two models of discharge planning rounds and compared their effects on the process of probing, identifying, planning for, and following through on patient discharge needs.
