[Acute blood purification for septic multiple organ failure].

Makoto Kobayashi1.

Abstract

Blood purification has been used for the prevention or treatment of septic multiple organ failure (MOF). Continuous renal replacement therapy (CRRT) is useful in the careful management of fluid, electrolytes, and acid-base balance and for the removal of metabolic wastes. Continuous plasma exchange (CPE) with CRRT is applied in order to cope with septic hepatic failure as an artificial liver support. Direct hemoperfusion with polymyxin B immobilized fiber (PMX-DHP) can rapidly improve circulation and decrease blood levels of cytokines. It is considered that these effects relate to PMX-DHP adsorbing endogenous cannabinoid. It is significant to judge the indication and combination of a suitable blood purification by the pathophysiologic causes of septic MOF.
