Abstract
PURPOSE: To report a case of clinical and anatomical reversal of long-term hypotony maculopathy after filtration surgery. DESIGN: Interventional/observational case report. METHODS: Case report and review of a 59-year-old woman who underwent surgical bleb revision for hypotony maculopathy. RESULTS: Over a 5-year period, progressive vision loss to 20/200 developed in the patient's right eye, with an intraocular pressure in the single digits. She underwent bleb excision, scleral patch graft, and conjunctival advancement. Five weeks later, her visual acuity had dramatically improved to 20/25 with an intraocular pressure of 16 mm Hg. The retinal thickness decreased from 423 microm to 211 microm based on ocular coherence tomography. CONCLUSIONS: This case demonstrates that long-term hypotony maculopathy is reversible both functionally and structurally.
