Abstract
The care of chronic kidney disease (CKD) patients is suboptimal. This article discusses how nephrology nurses can contribute to multidisciplinary clinics that work in collaboration with primary care providers to identify and manage patients with early CKD. This article will describe the importance of the nephrology nurse as a resource for improved patient outcomes and for the implementation of federal, professional, and corporate CKD initiatives.Entities:
