Abstract
This study presents case management as a care delivery model which is used in a variety of settings in the United States health care system. It basically designates a specific professional or health care team which is responsible for the care of an individual patient in the whole clinical process by coordinating care throughout services and institutions in the system. In nursing, the model has been accepted to increase the nurse's commitment in the standardization of practice. Central concepts include analysis, clinical decision-making and resolution of problems.Entities:
