Abstract
Situations often occur in anticoagulated patients that require adjustment of anticoagulation therapy. These situations often place the patient at risk of bleeding, and often there is little clinical data to guide the clinician. This paper reviews several of these situations and offers guidance for the management of patients. The problems reviewed concern the patient on anticoagulants who is or desires to get pregnant, peri-operative management of anticoagulated patients, anticoagulated patients with intracranial bleeding or endocarditis, and the "warfarin refractory" patient.Entities:
