Transition from Pediatric to Adult-Oriented Health Care for the Adolescent with Chronic Illness or Disability.

Adolescents and young adults with chronic conditions are given fewer opportunities to be independent health care consumers than their peers, and the transition from pediatric to adult-oriented care can be particularly difficult. For many chronically ill patients, transition medical services may be appropriate. The need for such services and ways to provide them are discussed.
